Animal Use Protocol Personnel Amendment Form
Amendments to Animal Use Protocols are submitted electronically through the Research Administration Management Portal (RAMP). RAMP functionality only allows for the submission of one amendment per protocol at a time. The following form has been developed to permit investigators with an amendment under ACUC review to add or delete protocol personnel. This form is only for use in the unique circumstance described. All other protocol personnel additions or deletions must be submitted through RAMP.
**Addition of personnel to an Animal Use Protocol requires ACUC approval. Following submission of this form, an automatic email acknowledgment will be sent. This email acknowledgment does not constitute ACUC approval. Once ACUC approval has been granted for the deletion or addition of persons listed below, a formal ACUC approval letter will be generated, and sent to the Principal Investigator or proxy. Personnel must not engage in animal work until formal ACUC approval has been received. Approved personnel deletions or additions will be manually entered into RAMP by ACUC personnel after the RAMP amendment in process has received ACUC approval.**
